Co-Management Resources

At the East Bay Eye Centers, we strive to provide you with a local, state-of-the-art ophthalmic referral center to assist you in the management of your patients’ eye care needs. We have established a long history of commitment to optometric education and co-management to help you provide the highest quality eye care to your patients.


Whether you seek small incision, clear corneal phacoemulsification with a multifocal implant and astigmatic correction, or a complicated glaucoma procedure, the doctors at the East Bay Eye Centers are available for your consultation and co-management.


In order to best assist your interactions with our office, we have prepared the following information to guide you. Here, you will find all the forms necessary for you to co-manage and bill Medicare for your participation in cataract surgery. You’ll also find the forms necessary to complete the co-management requirement for glaucoma. We strongly believe that co-management can provide the best all-around care for your patients, and in order for co-management to be successful, it depends on fluid and detailed communication between your office and ours. 

If you do not wish to co-manage your patients, the Doctors’, Severin, Edell, Raghu, and Feiz, are always available for consultation and management of most eye conditions. Since we do not dispense glasses at our office, your patients will always be returned to your facility for their annual eye examination and spectacle change.
